Pre-Op: Note central incisors with repaired fractured porcelain, front four teeth oversized relative to other teeth, excessive gingival display (UR). Patient had history of midline space that was minimized with two anterior bridges for missing lateral incisors without orthodontic intervention prior to case completion.
Post-Op: Finished case after orthodontics by Dr. Jeff Cohen and periodontal surgery / Fixed prosthodontics and advanced restorative by Dr. Rick Glassman.
Pre-Op: Note short right cuspid and midline shift to patient’s left.
Orthodontics, clear fixed banded appliance over older crowns, with sectioning of bridgework to allow for tooth movement.
Phase one provisional after orthodontics and periodontics to establish symmetrical guidelines.
Pre-Op: Upper right – Note gingival inflammation at #6 (canine).
Upper right orthodontic treatment – note severe proclination of front two central incisors, with plan to recline.

Three month post-op with ceramic try-in #3 (porcelain in-lay) and #4, #5 PFG(s).
